  • Call 999 in an emergency

Please contact Medicines Order Line on 0115 855 0260 or visit the NHS App or Systm Online for prescription requests. For urgent medication queries, please call 111.

If you had a scheduled routine appointment on 5th or 6th November, we will be in contact to rebook and reschedule.

You do not usually need proof of address, immigration status, identification or an NHS number to register with a GP practice.

  • Your name, date of birth and current address
  • Your previous GP practice, if you know it
  • Your NHS number, if you know it
  • Information about medicines, allergies and ongoing care
