Why does the healthcare navigator team need to ask what is wrong with me?
Our healthcare navigators are valuable members of the practice team, supporting clinicians. We have trained our staff to ask certain questions to help make sure that you receive the most appropriate medical care, from the most appropriate healthcare professional, at the most appropriate time.

Reasonable adjustments
Under the Equality Act 2010, organisations have a legal duty to make changes in their approach or provision to ensure that services are as accessible to people with disabilities as they are for everybody else. These changes are called reasonable adjustments.
At Gresleydale we want to ensure we are doing everything we can to make it easy for you to access health services. This may mean we need to make a reasonable adjustment for you.
A reasonable adjustment is a small change we can make to make appointments and communication easier for you.
You can do this by completing our Reasonable-Adjustment-questionnaire-2.docx and returning this to us.
Patients and carers
To find out more about the provision of reasonable adjustments, visit the Reasonable adjustments for people with a learning disability page on GOV.UK.
- Please tell us:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if you need an interpreter
- if you have any other access or communication needs
Home visits
If you are housebound and need an appointment, we will do a home visit. We may phone you first to understand what you need.
To request a home visit, it is helpful if you phone the practice as soon after 8am as possible please.
