Before filling out a registration form, check you are in our catchment area.

We are now registering patients residing at the new housing development in Drakelow, please use the NHS Register with a GP surgery service to register.

Registering with the Surgery

To ensure you have enough repeat medication, please request 4 weeks supply from your current surgery when starting your application.

You can register with the surgery using one of the following options:

Register online

1. Use the NHS Register with a GP Surgery Service to complete your registration online.
2. You will also need to download and print the New Patient Registration Questionnaire (PDF).
3. Fill in the questionnaire and hand it into the practice.

Print and complete the registration forms

1. Download and print both of these forms:
   • New Patient Registration Form (PDF)
   • New Patient Registration Questionnaire (PDF)
2. Complete both forms and hand the completed forms into the practice.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
