Practice Care Co-Ordinator

Lyndsey Titley – The Practice Care Coordinator is an administration post primarily trained to support GPs and the Clinical team: To address the ever-increasing demand for primary and secondary care services and better support patients within their own communities, there is a wide recognition of the need for local coordination of care needs for the practice population.

Patients represent a significant section that would benefit from an improved integrated and coordinated approach to ensure they receive the right level of support in the right place throughout their journey in primary care. To empower patients to actively engage in their treatment for better health outcomes.
