Dementia friendly practice

  • Gresleydale Healthcare Centre has recently received recognition from the Dementia Friendly South Derbyshire Society for the excellent work that has been achieved by the practice.

    The PPG fundraising team and staff donations have enabled us to purchase:

    • We endeavor to ensure that appointments are flexible and meet the needs of the patient.

    • We put appropriate consent procedures in place.

    • We us our computer systems to assist monitoring.

    • We respect carers as a key partner in the patient’s journey

    • We value the views of patients and carers within the practice

    • We provide appropriate dementia training for practice staff.

    • We commit to the development of a dementia friendly culture in the practice, holding events and provide a dementia friendly environment

    • We ensure the practice is part of a dementia friendly community.

    • We ensure patients receive a timely diagnosis.

    • We ensure care plans are integral to the overall care of the patient.

    • We ensure post-diagnostic support is a key component of follow-up care.

    • We ensure the physical environment and practice layout allows the patient to navigate the building independently.

    • We ensure patients are comfortable with and within the physical environment.

    We also run coffee mornings each year supported by the PPG, Age UK, and THE Derbyshire Carers association.
